OKT4抗体识别CD4，CD4是一种~59 kDa的单链1型跨膜糖蛋白，是免疫球蛋白（Ig）超家族的成员。CD4包含4个细胞外Ig样结构域（D1 - D4）。OKT4的表位定位于该蛋白的D3结构域，其结构类似于Ig可变区。CD4在大多数胸腺细胞和一些T细胞亚群（辅助T细胞）中表达水平相对较高，而在外周血单核细胞和巨噬细胞中表达水平较低。CD4结合MHC II的非多态性区域，并通过增强TCR与MHC II-抗原复合物之间结合的亲和力，在MHC II限制性抗原识别中作为T细胞受体（TCR）的共受体。CD4还通过胞内结构域与细胞质酪氨酸激酶（如Lck）的相互作用，放大从TCR到胞质的信号。此外，CD4是人类免疫缺陷病毒（HIV）的受体。OKT4抗体与CD4的结合不能阻断HIV结合。
